Frame sizes, and optimum encoding? Youtube sizes?

Since January I take it that this is now out of date:

In the YouTube player there is "360p", "480p", "720p", and sometimes "1080p".

What are the actual max width's and height's they're using here? I know that with "___p" it usually refers to the height.

I figure with 360p they must be referring to 480x360, and the 320x240 must be gone. With 720p they must be referring to 1280x720.

What is 480p? 640x480? 720x480? 854x480?

I also found this tool for finding out the best video bit rate:

I'm using the average settings, but with audio bumping it up to 96 and 44.1.

What is considered "good" and "standard" for web streaming?
